Job summary

Bridgeport Hospital is seeking a Registered Nurse to join our patient-centered care team. The RN will assess patient needs, plan and implement individualized care, coordinate care transitions, and provide health teaching to patients and families.

Qualifications

• Licensed Registered Nurse practicing under the Connecticut Nurse Practice Act.
• Experience delivering patient-centered care and health teaching.
• Strong communication with patients, families and the care team.
• Commitment to ethics, cultural awareness and professional governance.

Responsibilities

• Assess health status and nursing needs of patients.
• Plan, implement and adjust individualized care plans.
• Coordinate patient care and transitions across units.
• Provide health teaching, promotion, and infection prevention.
