Job summary

Hackensack Meridian Health seeks a licensed Physical Therapist Assistant to deliver patient-centered physical rehabilitation under supervision of a Physical Therapist. You will communicate regularly with PTs to ensure adherence to care plans and complete all required documentation in accordance with state and facility guidelines.

Qualifications

• Completion of an accredited program and licensed as a Physical Therapist Assistant.
• Current with all Continuing Education (CE) requirements
• Performs clinical care professionally
• Serve as a patient advocate and role model to other Team Members
• Deliver patient care with minimal disruptions and obstacles
• Ability to effectively work on a multidisciplinary team
• Excellent written and verbal communication skills.
• Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.

Responsibilities

• Demonstrates team approach to problem solving through frequent communication and collaboration on complex issues to provide appropriate care.
• Assists with discharge planning by collaborating with physicians, nurses, social workers, supervising physical therapist(s), and other healthcare providers.
• Instructs, encourages and assists patients to carry out plan of care, specific to patient¿s needs and goals established by Physical Therapist(s).
• Models and maintains safe environment to deliver services for patient, self, and healthcare team.
