We are currently hiring for the position of Patient Case Manager. This position will serve as a patient journey liaison for Intake, responsible for constant communication between departments for referral workflow. Creates professional interactions with internal and external customers and referral sources. Researches and identifies causes of slow referral movement and collaborates with other teams to improve the referral workflow process.
Education: High School Diploma or GED required. A current, unrestrictive Licensed Practical Nurse or Certified Pharmacy Technician for the State of Practice.
Experience: Minimal of 3 or more years of experience in a pharmacy, healthcare or clinical setting managing a high volume of complex patient cases withing an Intake/admissions referral process. Must be proficient in computer skills, including MS Office, Excel, and Word.
Functional Competencies: Knowledge and understanding of all aspects of the referral workflow process. Verbal and written communication, attention to detail, time management, initiative, prioritization, decision making, follow through, organization, multitasking, critical thinking, problem solving, dependability.
