PURPOSE AND SCOPE:
Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse, in accordance with organization policies, procedures, training, and in compliance with regulations set forth by the organization, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assists in maintaining a safe and clean working environment. Supports the organization’s commitment to the Quality Enhancement Program (QEP) and CQI activities, including those related to patient satisfaction, and actively participates in process improvement activities to help achieve the organization’s Quality Enhancement Goals (QEP).
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient-Related
Education:
- Assist healthcare team members in providing patient-specific education regarding adequacy measures such as Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and disease process/access.
Treatment:
- Welcome assigned patients, inquire about their wellbeing since last treatment.
- Evaluate vascular access for patency, perform cannulation, and administer heparin as delegated or allowed by law.
- Obtain pre- and post-treatment vital signs and weight; evaluate vascular access pre-treatment.
- Initiate dialysis treatment per prescribed orders, including blood flow (QB) and dialysate flow (QD).
- Evaluate intradialytic problems and provide interventions as prescribed or directed.
- Monitor patient responses to therapy.
- Obtain vital signs prior to reinfusion; perform all functions necessary for treatment discontinuation, document accordingly.
- Discontinue dialysis according to procedures, evaluate patient before venous access removal, including standing and sitting blood pressure.
- Obtain hemostasis, apply dressings, evaluate patient prior to discharge.
- Record pre- and post-dialysis evaluations, weight, vital signs with initial identification.
- Calculate and program dialysis machine settings for Ultrafiltration (UF) goals, treatment time, and UF modeling as prescribed.
- Ensure safety alarms are enabled, including Narrow Venous Limits (NVL) 160.
- Document treatment data accurately and timely onto the Hemodialysis Treatment Sheet with initial identification.
- Complete and document all treatment information, including progress notes.
- Report significant changes or patient condition to RN or supervisor.
- Conduct safety checks on machines, report anomalies.
- Intervene for unusual patient status, document responses.
- Monitor vascular access pressures, intervene as needed, report findings.
- Obtain lab specimens, ensure proper collection and handling.
- Clean blood spills with disinfectant per policy.
- Organize supplies efficiently to minimize waste.
- Supervise self-care under RN oversight.
Staff-Related
- Assist in training direct care staff under nurse supervision.
Education/Communication:
- Follow all organizational policies and procedures, including ethical practices.
- Maintain a safe, clean environment.
- Disinfect equipment and surroundings between treatments.
- Prepare for dialysis, test alarms, and initiate SDS system.
RECORD KEEPING:
- Participate in CQI activities.
- Enter treatment data accurately and promptly.
- Review treatment sheets for completeness, ensure signatures, correct omissions.
- Prepare admission charts and paperwork.
- Manage lab requisitions, collect, label, and store specimens properly.
INFECTION CONTROL:
- Assist with infection control audits.
- Support adherence to infection control practices.
- Follow policies, educate patients and families as permitted by law.
TRANSITIONAL CARE UNIT:
- Support new patients through initial 4 weeks of dialysis, coordinate care.
- Reinforce education, assist with home dialysis understanding.
- Coordinate with staff for patient care and education on treatment options.
- Assist with supply management for TCU.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
- Require passing Ishihara Color Blindness test; accommodations considered if failed.
- Involve desk work, patient interaction, travel between facilities, and community locations.
- Work environment is a healthcare setting with potential exposure to infectious materials.
- Require heavy lifting, moving patients, prolonged standing, bending, and occasional pushing/pulling up to 15-40 lbs.
EDUCATION AND LICENSES:
- High School diploma or G.E.D. required.
- Must meet CMS-approved certification requirements within the required timeline.
- Hold all necessary state licensure, education, and training.
- Demonstrate commitment to organizational values and customer service.
EXPERIENCE AND SKILLS:
- Preferred: prior patient care experience in a hospital or related setting.
- Successful completion of dialysis training and CPR certification required for continued employment.
