Job description

PURPOSE AND SCOPE:

Functions as part of the dialysis health care team in providing safe and effective home dialysis therapy and training for patients under the direct supervision of a licensed nurse, in accordance with organizational policies, procedures, training, and regulations set by the corporation, state, and federal agencies. Assists in setting up home dialysis equipment and maintaining a safe, clean environment within the home therapy program. Supports the organization's commitment to the Quality Enhancement Program (QEP) and CQI activities, including those related to patient satisfaction, actively participating in process improvement activities to help patients achieve the organization's QEP goals.

All duties and responsibilities of the PCT I HT are performed under the direct supervision of the Home Therapies registered nurse (HT RN), compliant with applicable laws, regulations, and policies.

PRINCIPAL DUTIES AND RESPONSIBILITIES (Not Home Assist):
  • Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law.
  • Identifies patients, obtains vital signs, weights, and completes the Clinic Visit Intake form.
  • Builds rapport and maintains communication with patients, care partners, HT RN, staff, providers, and team members.
  • Obtains and reviews treatment records, reports missing or abnormal data to the HT RN.
  • Assists with electronic treatment/medication reconciliation and reports issues to the HT RN.
  • May assist with home visits and provides home support as permissible.
  • Prepares supplies for laboratory testing, handles specimens, and assists with transplant specimen tracking and education.
  • Performs daily chores, disinfects equipment, prepares treatment rooms, stocks supplies, and manages inventory efficiently.
  • Schedules appointments, contacts patients/caregivers, and supports registration and connected health use.
  • Maintains logs, participates in audits, and helps with documentation and clerical duties.
Home Hemodialysis
  • Reinforces HHD training, sets up, programs, tests, disinfects machines, troubleshoots issues, and prepares dialysate.
  • Prepares supplies, evaluates access, initiates treatment, monitors patient status, and documents findings.
  • Performs safety checks, reports significant changes, and responds to events during treatment.
  • Ensures hemostasis, collects post-treatment vitals, assists with water testing, and performs other duties as assigned.
Peritoneal Dialysis
  • Reinforces PD training, troubleshoots cycler and connectivity issues, performs phlebotomy, and prepares solutions.
  • Assists with exit site care and PD equipment tracking, performs other duties as assigned.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
  • Includes desk work, patient interaction, travel, exposure to infectious materials, heavy lifting, prolonged standing, and physical exertion.
EDUCATION AND LICENSES:
  • High School diploma or G.E.D. required.
  • Must meet CMS-approved certification requirements and hold all necessary state licensure and training.
EXPERIENCE AND REQUIRED SKILLS:
  • Preferred experience in patient care settings; continued employment contingent on training completion and CPR certification.
