General Summary

Under the direction of professional nursing staff, provides direct patient care. Takes vital signs, checks surgical dressings, aids patients with activities of daily living. Performs blood draws and collects specimen samples. Observes and collects data, communicates patient status to identify needs, and provides necessary care. Serves as a member of the interdisciplinary patient care services team.

Principal Responsibilities and Tasks

The following describes the general nature and level of work performed by the person in this role. It is not an exhaustive list of all duties.

  1. Performs testing and manages patient care based on results and clinical findings.
  2. Records patient care activities, including ADLs: feeding, bathing, dressing, toileting, and transferring.
  3. Documents vital signs and pain levels.
  4. Assists with patient admission, transfer, and discharge.
  5. Participates in cost-effective strategies to optimize outcomes.
  6. Promotes patient safety, including fall prevention and maintaining a safe environment.
  7. Maintains order and appearance of patient rooms and work areas, including supplies and linen.
  8. Provides post-mortem care.
  9. Collaborates with the healthcare team to carry out the care plan and reports changes in patient condition.
  10. Ensures patient comfort and manages pain.
  11. Measures and documents urinary and stool output, calorie count, and patient weight.
  12. Assists with urinary catheter care, performs bladder scans, and documents findings.
  13. Conducts blood glucose monitoring.
  14. Performs venipuncture and establishes IV access.
  15. Performs nasal swabs.
  16. Prepares equipment and initiates IV placement as directed.
  17. Operates EKG machines and records rhythm strips.
  18. Assists with enteral feedings under supervision.
  19. Performs chest physiotherapy, tracheal suctioning, tracheostomy care, and incentive spirometry as delegated.
  20. Assists with dressing changes and wound care, reports skin integrity issues.
  21. Collects specimens for laboratory testing, ensuring proper identification and documentation.
  22. Provides orthopedic/rehab device care and applies/removes compression wraps or stockings.
  23. Precepts and mentors PCTs and manages documentation.
  24. Participates in hourly patient rounding.
  25. Responds to patient alarms and triggers.
  26. Follows safety protocols and reports adverse events.
  27. Provides vigilant patient care, including when acting as a safety companion.
  28. Uses safe lifting techniques and body mechanics.
  29. Improves patient flow and prevents delays.
  30. Performs other duties as assigned.
