Job summary

Jobtailor seeks a healthcare professional to provide patient care tasks across the life span in a clinical setting. You will assist, observe, and report patient changes to licensed caregivers and maintain accurate medical records.

Work under supervision providing direct patient care, including ADLs, vital signs, and documentation. Required: high school diploma or GED and completion of Block 1 in Nursing/MA/NA/EMT/BHT or 6 months healthcare experience; BLS certification; CNA certification

Qualifications

• High School Diploma or GED required.
• Block 1 completion in Nursing, Medical Assistant (MA), Nursing Assistant (NA), EMT, BHT, or 6 months healthcare experience required.
• 6 months acute care experience preferred.
• BLS certification required.
• CNA certification preferred.
• EMT certification/license preferred.

Responsibilities

• Provides patient care tasks and supports growth and development principles across patient lifespan.
• Observes, reports patient changes to licensed caregiver, maintains medical records.
• Delivers care under direct supervision of licensed caregiver.
• Performs ADLs, vital signs, intake/output, ambulation, and care plan implementation as directed.
• Reports changes in patient condition to healthcare team.
• Addresses questions or concerns from patients and families.
• Performs other duties as assigned.

Job description

• Provides patient related care tasks based on the principles of growth and development across the life span of patients served in his/her independent unit.


• Assists, observes, and reports patient changes to licensed caregiver and maintains accurate medical record documentation relative to the care delivered.


• Provides and collaborates direct patient care services under the direct supervisor of a licensed caregiver.


• Responsibilities may include, but are not limited to personal care, activities of daily living, vital signs, intake and output, ambulation, interventions, and implementation of selected portions of the patient care plan as directed under supervisor of nurse.


• Assists, observes and reports changes in patient condition to licensed caregiver and maintains accurate medical record documentation in relation to the care delivered.


• Addresses and/or directs questions or concerns from patients, families, caregivers, or health care team members.


• Performs other duties as assigned.

Requirements

• High School Diploma or GED Required


• Completion of Block 1 in Nursing OR Medical Assistant (MA) OR Nursing Assistant (NA) OR Emergency Medical Technician (EMT) OR Behavioral Health Technician (BHT) OR 6 months of experience in the healthcare setting within the last year Required


• 6 months in acute care experience Preferred


• Basic Life Support (BLS) - Certification Required


• Certified Nursing Assistant (CNA) - Certification Preferred


• Emergency Medical Technician - Basic (EMT) - License Emergency Medical Tech Preferred
