Live Well Homecare is seeking a healthcare professional to provide patient care under the supervision of a Registered Nurse. The role includes monitoring vitals, conducting EKGs, bladder ultrasounds, glucose checks, and specimen collection, while assisting with activities of daily living.

The position offers full-time and part-time schedules across various Illinois locations. Overnight and weekend shifts with differential are available, with a comprehensive benefits package upon employment.

Qualifications

• Current CPR/BLS certification is required.
• 1+ year of relevant medical experience is required.
• Basic computer proficiency for charting and documentation is required.
• Strong interpersonal, prioritization, collaboration, and compliance skills are essential.

Responsibilities

• Monitor vital signs and assess patient condition.
• Perform 12-Lead EKGs and bladder ultrasounds as assigned.
• Conduct blood glucose checks and specimen collection.
• Assist patients with activities of daily living (bathing, feeding, turning, ambulation).
• Assist with admissions and discharge preparations and implement care plans.
• Maintain hourly rounding and follow safety protocols; provide emotional support to patients.

Responsibilities include monitoring vital signs, performing 12-Lead EKGs, bladder ultrasounds, blood glucose checks, specimen collection, and assisting with patient activities such as bathing, feeding, turning, and ambulating.
Assist with patient admissions, discharge preparations, and implementing care plans. Maintain hourly rounding, support safety protocols, and offer emotional support to ensure a safe, high-quality patient experience.

Qualifications & Skills

• Current CPR/BLS certification required
• Completion of CNA training or nursing clinical rotation preferred
• 1+ year relevant experience in a medical setting required
