Join to apply for the Patient Care Specialist - SBHC PCMS role at Open Door Family Medical Center.

Open Door Family Medical Centers has been serving low-income, under-served residents and those lacking access to health care in Westchester and Putnam counties for over 40 years. We provide high-quality primary health care and human services at affordable prices. This year, our medical home will serve more than 50,000 patients with over 250,000 visits. Our services include primary medical care, dentistry, mental health, optometry, podiatry, and women’s health across Ossining, Sleepy Hollow, Port Chester, Mount Kisco, and Brewster, NY. We are seeking a Patient Care Specialist for our School-Based Health Center at Port Chester Middle School, NY. A certification from an accredited health care agency is required, along with at least 1+ years of experience as a medical assistant. Bilingual English/Spanish skills are necessary. The position may require local travel to other school-based program locations in Port Chester, NY.

Responsibilities
  1. Perform and accurately record patient’s chief complaints, medical history, screening tests, vital signs, vision, hearing, EKG, and spirometry for pediatric and adolescent patients.
  2. Assist providers with examinations, diagnostic, and therapeutic tests.
  3. Provide patient-centric care with empathy, active listening, and effective communication.
  4. Document necessary information on patient’s electronic records, including referrals and medical forms.
  5. Update practice management and electronic medical record systems with clinical information before the end of each day, complying with TJC standards.
  6. Participate in patient counseling and education on primary health care topics such as HIV/STD prevention, health maintenance, and family planning.
  7. Perform simple laboratory procedures like rapid strep tests, urinalysis, pregnancy screening, hemoglobin A1C, blood glucose monitoring, and venipuncture, ensuring proper calibration, cleaning, and maintenance of instruments.
  8. Collect and label laboratory specimens, including blood, stool, and urine.
  9. Reinforce patient and family education initiated by professional staff.
  10. Handle medical clerical operations, including registration, appointment scheduling, check-in/out, claims creation, and medical record retrieval.
  11. Retrieve patients from classes or other school areas for appointments during school hours.
  12. Communicate with school nurses, teachers, administrators regarding health center operations and patient needs.
  13. Communicate with parents, including translation support when needed, as directed by the provider.
  14. Maintain strict confidentiality of sensitive information, in accordance with privacy laws.
  15. Manage patient-technician relationships effectively.
  16. If responsible for biohazardous waste removal, complete required training and obtain certification prior to performing this task.
Requirements
  1. High School Diploma or GED.
  2. Accredited Medical Assistant certification is required.
  3. Bilingual in English and Spanish.
  4. Superior customer service skills and experience.
  5. At least 1+ years of experience as a Medical Assistant.
  6. Flexible, solutions-oriented, and able to work in a fast-paced, high-accountability environment.
  7. Compassionate, cheerful, and concerned in patient care.
  8. Comfortable engaging with adolescents professionally and warmly.
  9. Willing to travel periodically to other school-based health centers in Ossining and Port Chester.
  10. Passionate about serving low-income, underserved populations.
  11. Maintain patient confidentiality at all times.
  12. Committed to learning and quality improvement.
  13. Excellent verbal and written communication skills with diverse populations.
  14. Proficient in MS Office and other data systems.
  15. Able to move around for long periods and perform duties involving bending, lifting light items, and repositioning patients.
