Elara Caring in Dallas, TX is seeking a Patient Care Representative to support prior patients, coordinate with internal teams, and help grow our home health care services. The role emphasizes clear communication, data accuracy, and reliability in daily case management.

You will report to the Program Supervisor, work with MS Office tools, and participate in trainings. The position requires a High School Diploma or GED and 1+ year of home health care experience preferred; benefits include

• Participate in trainings, seminars, and quality improvement activities.
• Assist in scheduling and coordinating patient care activities and care transitions.
• May be required to work weekends and holidays as assigned.
