Coordinates and supports individual patients and families throughout point of care program(s) to ensure program goals are effectively executed with some guidance. Assures coordination of diagnostic evaluations, treatments, surgeries and follow-up appointments throughout the course of care/episode for a specific patient population.

The Patient Care Program Coordinator (PCPC) supports both the Colorectal Surgery and General Surgery service lines, serving as a key liaison between patients, providers, and care teams. This hybrid role requires a highly organized, self-motivated individual who can effectively manage complex patient care coordination needs while working both independently and collaboratively. The ideal candidate is a high-performing individual with exceptional patient communication skills, the ability to build trust and provide compassionate support, and a proactive approach to problem-solving. This position requires resourcefulness, sound judgment, and the ability to independently navigate barriers to care, coordinate services, and ensure a seamless patient experience into the surgical clinics.
Supports patient care coordination across both Colorectal Surgery and General Surgery practices. Functions effectively in a hybrid work environment. Demonstrates strong verbal and written communication skills with patients, families, and healthcare teams. Independently identifies and resolves issues impacting patient access, scheduling, referrals, and care coordination. Highly organized, adaptable, and able to manage multiple priorities in a fast-paced clinical setting. Resourceful and self-directed, with the ability to make informed decisions and elevate concerns when appropriate. Committed to delivering excellent patient service and supporting positive patient outcomes.
