Assures coordination of diagnostic evaluations, treatments, surgeries and follow-up appointments throughout the course of care/episode for a specific patient population..**Position Hours**Monday - FridayDaysThe Patient Care Program Coordinator (PCPC) supports both the **Colorectal Surgery** and **General Surgery** service lines, serving as a key liaison between patients, providers, and care teams. This hybrid role requires a highly organized, self-motivated individual who can effectively manage complex patient care coordination needs while working both independently and collaboratively. The ideal candidate is a high-performing individual with exceptional patient communication skills, the ability to build trust and provide compassionate support, and a proactive approach to problem-solving.
