Job summary

Kaweah Health seeks a Pharmacy Technician to support outpatient clinical pharmacy services under a clinical pharmacist. You will collect and document medication histories, ensure accurate records, and assist with patient onboarding and education in a fast-paced setting.

Qualifications include California Pharmacy Technician registration, at least one year in a retail or medical setting, and proficiency with pharmacy software and EMR systems. Bilingual Spanish encouraged.

Qualifications

  • Current California Pharmacy Technician or Intern Pharmacist registration
  • Preferred: National Board Certification as Pharmacy Technician
  • High school diploma or equivalent required
  • Bachelor's Degree preferred
  • One year of prior work experience in retail pharmacy or medical setting with medication reconciliation
  • Knowledge of outpatient brand and generic medications and prior authorization processes

Responsibilities

  • Collects and prepares patient care notes and medication histories for clinical pharmacy visits
  • Documents complete medication histories in the electronic medical record
  • Maintains accurate list of home medications including supplements and ACHHI details
  • Processes current medications into the medical record with complete prescription information
  • Interviews patients and care teams to clarify medication information
  • Provides patient education materials and follow-up plans after visits
  • Schedules appointments and reinforces adherence and self-management instructions
  • Participates in training and supports new staff

Under the direct supervision of the Clinical Pharmacist, the Patient Care Pharmacy Technician will perform assigned tasks necessary to support comprehensive clinical pharmacy services provided by outpatient Clinical Pharmacists.

QUALIFICATIONS
License /Certification
  • Required: Current California Pharmacy Technician or Intern Pharmacist registration
  • Preferred: National Board Certification as Pharmacy Technician
Education
  • Required: High school diploma or equivalent
  • Preferred: Bachelor's Degree
Experience
  • Required: One year of previous work experience (prefer retail pharmacy or medical setting performing medication reconciliation).
Knowledge/Skills/Abilities
  • Knowledge of outpatient brand and generic medications, prior authorization processes and patient assistance programs.
  • Proficient with Microsoft Word, Excel and pharmacy software.
  • Ability to work in multiple electronic documentation systems.
  • Strong communication skills and desire to work in direct patient care.
  • Bilingual in Spanish preferred.
Department Specific Requirements
  • For Pharmacy employees assigned to work in the Emergency Dept: requires Kaweah Health approved crisis intervention training within 60 days of hire/transfer.
JOB RESPONSIBILITIES
Essential
  • Collects, organizes and prepares patient care notes, profiles and other clinical information for clinical pharmacy appointments and medication reconciliation.
  • Provides technical assistance with collecting, clarifying and documenting complete and accurate medication histories under the direction of the clinical pharmacist.
  • Compiles a complete and accurate list of the patient's home medications including over-the-counter medications and herbal supplements inclusive of the drug/product name, dosage, frequency, route, indication, compliance, drug allergies and symptoms.
  • Processes/enters current medications with complete prescription information into the computerized medical record.
  • Clarifies the medication information through interview with the patient, family, care givers, pharmacies, previous medical records and medical providers as necessary.
  • Ensures patients leave clinical pharmacy visits with appropriate education materials, follow-up plans, and return appointments as appropriate.
  • Contacts select patients by phone to schedule appointments, confirm whether treatment goals are being met and reinforce key self-management instructions and medication adherence under the instruction and supervision of the clinical pharmacist.
  • Reviews registries and other reports to identify targeted patients for pharmacist intervention.
  • Administers quality of life or disease control surveys and patient satisfaction surveys.
  • At the direction of the clinical pharmacist, facilitates the enrollment of patients into medication Patient Assistance Programs (PAP), including KH 340B pharmacy program.
  • Assists in the collection of health outcome data and assemble routine reports for clinical pharmacists, administrators, medical staff, and other stakeholders.
  • Inspects medication storage areas according to procedures.
  • Participates in Pharmacy staff meetings
  • Guides, directs, and assists in training new staff.
Addendum (essential for specific dept)
IF ASSIGNED TO WORK AT SRCC-MEDICAL ONCOLOGY ALSO RESPONSIBLE FOR:
  • Performs time-sensitive pharmacy based patient services such as patient onboarding, benefit verification, prior authorization, and appeals, as needed, on behalf of patients and their physician to facilitate the process between the doctors' office and pharmacy to triage and streamline prescription fulfillment outpatient pharmacy dispensing.
  • Provides above services for a health system's ambulatory pharmacy, specialty pharmacy and/or outpatient infusion center environment, as requested.
  • Verifies benefits for pharmacy coverage of medications.
  • Interacts with patients and providers to obtain insurance and patient information needed to request prior authorizations so that medications and other services rendered are reimbursed by payer.
  • Researches various payers to understand their prior authorization and appeals processes.
  • Responsible for facilitating patient enrollment into manufacturer and/or foundation sponsored patient assistance programs as they relate to pharmacy benefits.
  • Demonstrates flexibility and adapts to changes in payer, pharmacy and patient assistance program processes and requirements.
  • Establishes client relationships with assigned clinic staff, pharmacy and hospital contacts, maintaining professional communications at all times and keeping them informed of patients' status and payer challenges.
  • Communicates patient and provider feedback to management to enhance process improvement and continuity of care of services provided.
  • Provides coverage to support clinical pharmacy operations, including compounding responsibilities, if assigned and as appropriate, through concurrent job responsibilities as Pharmacy Tech II.

Pay Range

$25.28 -$37.92
