Qualifications

Responsibilities

Position Summary

The Patient Care Partner provides direct patient care under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). This role supports patients' physical, emotional, and psychosocial needs by assisting with daily care activities, monitoring patient conditions, promoting safety, and ensuring a positive patient experience.

Schedule

Per Diem (PD)

  • Flexible scheduling based on department needs
  • Weekend, holiday, and variable shift availability may be required
Responsibilities
  • Provide direct patient care including bathing, dressing, grooming, toileting, feeding, and oral hygiene.
  • Assist patients with mobility, transfers, ambulation, positioning, and range of motion exercises using Safe Patient Handling equipment and established mobility protocols.
  • Obtain, document, and report vital signs, heights, weights, intake/output, bowel movements, and activities of daily living (ADLs).
  • Collect urine, stool, and sputum specimens while educating patients on proper collection techniques.
  • Perform delegated clinical duties including EKGs, simple dressing changes, surgical preps, incentive spirometry supervision, Foley catheter removal, peripheral IV catheter removal, cleansing enemas, and postmortem care, as appropriate.
  • Provide one-to-one observation for patients requiring continuous monitoring due to confusion, behavioral concerns, or suicide precautions.
  • Respond promptly to patient call lights and participate in hourly rounding to promote patient safety and satisfaction.
  • Transport patients, equipment, and blood products safely throughout the facility.
  • Maintain a clean, organized, and safe patient environment, including equipment management and patient belongings.
  • Recognize changes in patient condition and promptly communicate concerns to the nursing team.
  • Support patient safety initiatives, quality improvement activities, and departmental performance goals.
  • Demonstrate professional, compassionate communication with patients, families, physicians, and interdisciplinary team members.
  • Assist with orienting new staff members and contribute to a positive team environment.
  • Maintain CPR certification and comply with all hospital policies, procedures, and regulatory requirements.
Qualifications
Education
  • High School Diploma or equivalent required.
Experience
  • Successful completion of a Certified Nursing Assistant (CNA) training program; or
  • Minimum one (1) year of experience as a Nurse Aide or equivalent patient care role;
  • Current enrollment in an accredited nursing program with successful completion of a Fundamentals of Nursing course.

  • Ability to lift and carry up to 50 pounds regularly and assist with moving patients weighing 100+ pounds.
  • Frequent exposure to blood, body fluids, communicable diseases, and other healthcare-related hazards.
  • Ability to work in environments involving body odors, bodily fluids, and emotionally challenging patient situations.
