Job summary

Qualifications

• Bachelor's degree in healthcare or related field; total 4 years of relevant experience.
• Experience in direct patient care, quality improvement, or health coaching preferred.

Responsibilities

• Acts as liaison between member/family, physician and facilities.
• Educates providers on HEDIS measures and ICD-10/CPT coding per NCQA.
• Schedules appointments for care gaps and coordinates transportation as needed.
• Documents all actions and coordinates with other departments to resolve issues.
