Qualifications

• Bachelor's degree required with total 4 years of experience in direct patient care, social work, quality improvement or health coaching (2+ years in direct patient care preferred).
• Licensure such as LPN, LMSW, CSW, LSW, RN preferred.

Responsibilities

• Close care gaps by outreach and in-provider visits to improve HEDIS measures.
• Educate providers on HEDIS coding and NCQA requirements.
• Schedule appointments and coordinate wraparound services for members.
• Assist in arranging transportation and connecting with community resources.
• Maintain confidentiality of PHI and communicate across departments to resolve issues.

Education

Bachelor's Degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related health field
