Job summary

Munson Healthcare is seeking a Patient Care Manager in northern Michigan to oversee progression‑of‑care, monitor patient outcomes, and advocate for appropriate care levels. The role emphasizes relationship‑based care, collaboration with physicians, nurses, social workers, and discharge planning to ensure timely, high‑quality care.

The position requires a nursing or social work background, an MSW or related license, and hospital‑based patient care experience.

Qualifications

• Bachelor’s Degree in Nursing or Social Work; equivalent experience may be considered (RN or social work license required).
• MSW license required; LMSW considered.
• This position requires at minimum a Limited Master Social Work license in Michigan.

Responsibilities

• Facilitate progression‑of‑care and monitor patient progress toward the plan of care.
• Support RBC principles and unit action plans.
• Maintain knowledge in utilization management, managed care, and payer issues affecting care.
• Respond promptly to referrals for case management services.
• Identify community resources and collaborate with patients, families, and multidisciplinary teams.
• Advise on admission diagnosis and related quality/care metrics to promote medical compliance.
• Advocate for patients and ensure appropriate levels of care.
• Educate healthcare team on access to and use of care levels.
• Collaborate with discharge planners and post‑acute coordinators to ensure timely discharge and post‑discharge linkage.
• Interface with utilization review to determine eligibility for admission and discharge.
• Participate in quality improvement activities and other department initiatives.

Education

Bachelor’s Degree in Nursing or Social Work

MSW license (LMSW considered)

Limited Master Social Work license (Michigan)
