The Patient Care Manager Assistant supports the Patient Care Manager by coordinating staff to ensure high-quality home health care. Responsibilities include ensuring staff are skilled, informed, and providing care in accordance with physician's orders and regulations.

• Assist with communication between staff, physicians, and patients
• Monitor incoming faxes, labs, and hospital status for care coordination
• Manage reports for care coordination and utilization management
• Assist with authorizations and follow-up on health alerts
• Prepare documentation for case conferences
• Provide direct patient care as needed, within scope of practice
• Support documentation review and audits

License Requirements:

• Current CPR certification
• Current LPN licensure in the state of practice
• Valid driver's license, vehicle insurance, and access to transportation
