Responsibilities

The Home Health Patient Care Manager is responsible for supervising and coordinating clinical services, providing and directing nursing care to patients in their homes as prescribed by physicians and in compliance with laws, regulations, and agency policies.

Provides clinical services within the scope of practice defined by state laws, in accordance with the care plan, and in coordination with the healthcare team.

Receives referrals, assigns clinicians appropriately, evaluates patient orders, and initiates start of care visits.

Coordinates home health benefits, medical necessity, and insurance approvals.

Ensures patient needs are continually assessed, care is individualized, appropriate, meets eligibility criteria, and follows physician orders.

Reviews assessments and plans of care daily, consults with clinicians as needed.

Education and Experience

• Current RN licensure in the state of practice
• Current CPR certification required
• Valid driver's license, vehicle insurance, and access to reliable transportation
