Responsibilities

The Home Health Patient Care Manager and RN Dual Role supervises and coordinates clinical services, providing nursing care to patients in their homes as prescribed by physicians and in compliance with laws, regulations, and agency policies.

• Provides clinical services within the scope of practice, as defined by state laws, in accordance with the care plan, and in coordination with the healthcare team.
• Receives referrals, assigns clinicians, evaluates patient orders, and schedules start of care visits.
• Coordinates patient home health benefits, medical necessity, and insurance approvals.
• Ensures ongoing assessment of patient needs, individualized care, and compliance with physician orders and eligibility criteria.

Reviews assessments and care plans daily, consults clinicians with recommendations as appropriate.

Education and Experience

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to reliable transportation
