Job summary

Heart to Heart Hospice is seeking a Patient Care Manager - Registered Nurse to lead clinical departments and sites, ensuring high-quality, compliant patient care with a strong focus on safety and outcomes.

The role requires a licensed RN with hospice experience and the ability to supervise and coach nursing staff within a multidisciplinary team, while maintaining thorough documentation and adherence to regulations.

Qualifications

• Current State Registered Nurse license.
• Valid driver's license with reliable transportation and auto liability insurance.
• Two years of hospice RN experience in clinical care or home health.
• Able to work in a multidisciplinary group setting.
• Strong organizational, interpersonal, and communication skills.
• Ability to multi-task multiple projects and strategies.

Responsibilities

• Direct and coordinate clinical departments and sites; ensure continuity, quality, and safety in compliance with regulations (Conditions of Participation).
• Supervise and provide direction to nursing staff to ensure quality and adherence to the Plan of Care and patient needs continuity.
• Support employee and patient education to enhance knowledge and skills; train nursing staff for high-quality patient care.
• Provide coaching, development, feedback and annual evaluations to team members in a professional manner.
• Timely submission of all documentation.
• Perform other duties as required.

What You Must Have:

• Current State Registered Nurse License
• Current State Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as a Hospice Registered Nurse in a clinical care setting, or home health
• Must be highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary group setting
• Possess good organization, people and problem solving skills and the ability to multi-task many projects and strategies simultaneously
• Strong critical thinking, organizational, interpersonal, and communication skills

What You Will Do

• Direct and coordinate clinical departments and sites; assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations (Conditions of Participation).
• Supervise and provide direction to nursing staff and team members, in an effort to ensure quality, compliance with Plan of Care, assessment and reassessment of patient's needs and continuity of services by appropriate health care personnel.
• Supports employee and patient education to enhance knowledge, skills and achieve quality experiences. Continuously trains nursing staff to retain employees with the highest quality of patient care.
• Provide coaching, development, feedback and annual evaluations to team members in a professional and timely manner.
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
