️ About the Role

As a Patient Care Manager, you will play a key leadership role in supporting clinical staff, guiding patient care decisions, and collaborating with interdisciplinary team members to ensure our patients receive compassionate, expert care tailored to their needs.

Work Schedule:

Full-time, Monday through Friday

Some after-hours coordination may be required

Key Responsibilities

  • Supervise and support nurses, aides, social workers, counselors, and therapists
  • Collaborate with the Director of Clinical Services to implement clinical policies and procedures
  • Support quality care delivery through ongoing staff guidance and performance improvement initiatives (QAPI)
  • Assist in complex patient care situations and help adjust care plans as needed
  • Coordinate physician referrals and ensure proper clinical communication
  • Oversee accurate and timely documentation, time sheets, and required personnel reports
  • Ensure 24/7 hospice nursing coverage through on-call scheduling
  • Maintain availability of essential medical supplies for staff and patient care


What You Bring

  • Current Registered Nurse (RN) license in the state of New Jersey
  • CHPN certification preferred but not required
  • At least 2 years of hospice and/or home health experience preferred
  • Strong knowledge of hospice philosophy, symptom management, and end-of-life care
  • Excellent communication, organizational, and leadership skills
  • Valid driver’s license, auto insurance, and reliable transportation
  • Must successfully pass background checks, drug screening, and reference verification

  • Job Type: Full-Time, Salaried
  • FLSA Classification: Exempt
  • Compensation: The annual salary range for this position is $90,000 – $105,000, commensurate with experience.
