Qualifications

• Bachelor's degree or equivalent experience in a clinical setting.
• Minimum 1–2 years in a clinical care environment, ambulatory or community health center preferred.
• Proficient with EMR, Excel, Outlook, Word and general office software.
• Attention to detail and effective written and verbal communication.

Responsibilities

• Prepare for patient visits by gathering information and ensuring external records are received.
• Support clinical visits through EMR note preparation for various exams and care visits.
• Follow up with recently discharged patients to ensure continuity of care.
• Coordinate DME orders and documentation in the EMR.
• Enter orders into EMR as directed by clinicians.
• Review schedules and ensure external documents are received (discharge summaries, labs, etc.).
• Assist administrative tasks and care-gap closures under supervision.
