Job summary

West Coast Wound is seeking a Patient Care Coordinator to oversee daily clinical and operational functions supporting the Medical Programs. You will provide leadership, mentorship, and ensure adherence to clinical protocols and documentation standards.

You will supervise staff, coordinate coverage, manage patient follow-up, and maintain accurate records in a fast-paced wound care environment. Strong communication and EMR proficiency are essential.

Qualifications

• High School Diploma or GED required.
• Associate degree or related healthcare field preferred.
• Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or equivalent certification preferred.
• Current CPR/BLS certification preferred.

Position Summary

The Patient Care Coordinator is responsible for overseeing the daily clinical and operational functions supporting the Medical Programs. This role provides leadership, guidance, and mentorship to the team while ensuring compliance with clinical protocols, regulatory requirements, and documentation standards. The Patient Care Coordinator serves as the primary liaison between program staff and clinical leadership, functioning as both a frontline supervisor to maintain high standards of patient care, workflow efficiency, and patient satisfaction.

Essential Duties and Responsibilities

Leadership & Team Management

• Supervise and support in the day-to-day execution of program responsibilities.
• Orient, train, and onboard outsource staff on program-specific workflows, clinical protocols, and documentation requirements.
• Conduct or contribute to employee performance evaluations and identify opportunities for professional development and continuing education.
• Coordinate and ensure appropriate coverage across all Medical Programs.
• Serve as the first point of escalation for staff questions, workflow concerns, and patient-related issues.
• Lead regular team huddles and participate in departmental meetings and quality improvement initiatives.
• Foster a collaborative, professional, and accountable team environment.

Clinical & Administrative Oversight

• Oversee timely patient follow-up and continuity of care.
• Maintain accurate treatment logs, measurement tracking, and prior authorization documentation.
• Ensure accurate and complete ICD-10 coding and clinical documentation.
• Monitor compliance with organizational policies, regulatory requirements, and documentation standards.
• Assist with resolving operational issues and proactively communicate supply needs and workflow concerns to the Program Director.

Quality & Compliance

• Ensure adherence to established clinical protocols and best practices.

• Support quality assurance initiatives and process improvement efforts.
• Monitor documentation of accuracy and completeness to support compliance and reimbursement requirements.

Education & Certifications

• High School Diploma or GED required.
• Associate degree or related healthcare field preferred.

• Current CPR/BLS certification preferred.

Experience & Qualifications

• Minimum of three (3) years of clinical Medical Assistant experience required.
• Minimum of one (1) year of wound care or specialty clinic experience is strongly preferred.
• Previous lead, charge, or supervisory experience preferred.
• Demonstrated ability to lead, coach, mentor, and hold team members accountable in a professional and respectful manner.
• Strong organizational, time management, and multitasking abilities in a fast-paced clinical environment.
• Excellent verbal and written communication skills.
• Proficient in electronic medical records (EMR) systems and healthcare documentation practices.
