As a Patient Care Coordinator at Arcare, you will play a vital role in ensuring comprehensive care coordination for HIV/AIDS clients, combining traditional case management with enhanced patient coordination services. The PCC-Infectious Disease will work closely with patients, their families, healthcare providers, and other stakeholders to facilitate communication, manage care plans, and provide necessary resources, referrals, communication, behavioral health, coordinated care, and insurance.
