Job summary

Avera eCARE is seeking a Patient Care Coordinator to promote home healthcare and assist with patient transitions from health facilities to home. Qualifications

  • Must have a valid Driver's License.

A Brief Overview

Accountable for working in partnership with facilities, discharge planners, clinic staff and healthcare workers to promote home healthcare, hospice and Care Transitions with the goal to provide a smooth transition from the acute, skilled nursing facility and clinic setting to the home. Builds relationships with referral sources to provide education on what Avera can offer their patients. Assists in providing education to patients, families and facilities on what home health, hospice and Care Transitions programs are. Once reviews patient's qualifications and initiates referral to designated program.

What you will do

  • Attends multidisciplinary and discharge planning meetings to assess, plan and evaluate discharge plan for home care needs.
  • Functions as a liaison between health care facilities, clinics, and physicians in the coordination of discharge planning needs for homecare services.
  • Serves as a resource to area healthcare facilities, physicians, discharge planning and other staff regarding home care and hospice services.
  • Provides education to staff in relation to home healthcare, hospice and home medical equipment services.
  • Knowledge and skills sufficient to integrate the use of health care technology and/or information management systems into the referral process for patients.
  • Interviews patients/families in the hospital, nursing home and clinic to assess and evaluate patient for home care appropriateness.
  • As part of the initial referral evaluation, verifies home health and hospice benefits according to payor plans.
  • Initiates referral process and interdisciplinary communication.
  • Coordinates the completion for homecare referral forms, releases, face to face, physician orders, and other documentation for homecare or hospice.

Required Education, License/Certification, or Work Experience:

  • Drivers License - Licensing Board Upon Hire

Preferred Education, License/Certification, or Work Experience:

  • Bachelor's in Healthcare, Sales, or Business
  • Training and experience in health care related field, home health or hospice.
