Porch Light Health in Pueblo, Colorado seeks a dedicated Patient Care Coordinator to join our fast-paced medical team in-person at the Pueblo Clinic and Mobile Unit. You will support professional staff by scheduling patients, taking vitals, answering calls, and guiding patients through admission and registration processes.

Ideal candidates have experience in medical clinics or addictions treatment, basic computer skills, reliable transportation, and strong time management.

Responsibilities

• Scheduling patients and coordinating appointments.
• Collecting vitals and patient information.

• Maintain building and cleaning and liaise with medical staff.
• Provide patient-centered care and strong customer service.
• Demonstrate understanding of MAT, Harm Reduction and safety practices.

Job Summary

We are seeking a dedicated and compassionate Patient Care Coordinator to join our growing team in a fast-paced, dynamic medical setting. We are looking for people who are willing to work in a fast paced medical environment.

This is a rover position- you will be required to rover to multiple rural locations within the area.

Responsibilities and Duties

This Is The Entry Level Position Where The Focus Of Training Is On Developing And Demonstrating Competencies Of Supporting Professional Staff By

• Scheduling patients
• Collecting vitals and other information
• Answering phones
• Directing patients
• Admitting and registering patients
• Data and record keeping
• Building maintenance and cleaning
• Communication with medical staff
• patient centered care and customer service
• Additionally staff will need to demonstrate a high-level, broad understanding of MAT, Harm Reduction, Low Barrier/High Access, HIPAA, OSHA and safety practices, verbal de-escalation and crisis intervention.

Qualifications And Skills

• Previous medical clinic and/or addictions treatment experience preferred
• In some cases experience can be substituted for education requirement
• Basic computer skills
• Reliable transportation

Job Type: Full-time

Pay: $19-24 based on education, experience and location

• 8 hour shift, 8:00am-4:30pm M-F
• Saturday Rotation

• High school or equivalent (Preferred)
