Position Summary

S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Patient Care Coordinator’s primary function is to schedule patient services and coordinate with staff.

Position Specific Responsibilities

• Schedules and appropriately documents patient schedules in a timely manner.
• Participates in coordinating care with management and patient interdisciplinary team.
• Provides effective communication to patients, staff members, other health care professionals, and referral sources.
• Responds to agency and patient needs in a professional and creative manner.
• Performs on-call responsibilities as assigned.
• Performs other duties as assigned.

Education and/or Experience

• High school diploma or GED required.
• Experience with patient scheduling highly preferred.
• Prior customer service preferred.

Compensation

The current compensation range for this position is $19.72-$30.1 per hour. The specific rate of pay offered to a selected candidate will be determined based on relevant education, training, experience, geographic location, and other position-related factors. Compassus does not inquire about, consider, or rely upon an applicant’s prior salary history in determining compensation. In addition to base compensation for the position, an employee working in this position may be eligible for pay differentials based on hours, shifts worked, or other criteria. They may also qualify for bonuses under applicable bonus programs or at Compassus’s discretion, as well as other forms of variable pay under relevant company policies.A summary of applicable pay programs will be provided during the offer process.
