Summary of Primary Function/General Purpose of Position:
Provides continuous observation and surveillance of assigned patient(s) to promote and maintain patient safety.Performs various duties to provide care and comfort for assigned patient(s).Recognizes and communicates pertinent patient information to nursing staff, coordinators, and supervisors to ensure prompt attention to and resolution of issues and concerns.
Essential Job Functions:
Provides continuous visual observation and surveillance of patient(s) on monitor or at bedside to promote and maintain patient safety.
Verbally redirects patients either directly or over audio device, activating alarms as indicated.
Identifies, troubleshoots, and works with others as applicable to resolve issues and concerns.
Documents patient activity, behaviors, and interventions per departmental policy.
Assist with activities of daily living, including but not limited to bathing, feeding, toileting, dressing, grooming, and transportation.
Participates in hand off of pertinent information regarding assigned patient(s) upon arrival and when leaving.
All other duties as assigned.
Acts as liaison between patient and nurse to report changes or concerns to nurse-
Provides high level customer service to all patients, patient's family, visitors and all employees
Provides accurate, precise, timely documentation when applicable for patient care
Uses professional, verbal and written communication skills at all times
Work Experience:
Preference for recent experience in healthcare or related field.
Required Skills:
Ability to clearly communicate to other members of the healthcare team
Ability to understand and follow directions of healthcare team
Knowledge of patient and healthcare safety standards and regulations (i.e. falls, skin breakdown, suicide prevention, environment of care)
Infection prevention and transmission based precautions
Understanding of clerical duties and office based technology
Team dynamics
Patient data collection and documentation (i.e. observations, intake/output, change in patient condition)
Basic hygiene and activities of daily living care
Feeding, hydration, and nutrition
Bowel and bladder elimination
Ambulation and patient mobility
Use of clinical technology
Compassionate, relationship based approach in care activities
Accountability for completion of assigned tasks
Escalation of concerns via chain of command
Certifications:
BLS Basic Life Support - American Heart Association (preferred)
Education
High School/GED (required)
Hours:
Shift: ALL
Hours: 12 Hour Shifts
Hours per pay period: VARIES
Weekend, holiday and on-call rotation required per department policy
