Job Description

PURPOSE OF POSITION

Under the supervision of a licensed Nurse or other professional medical staff, performs essential health-related duties related to patient care, clerical tasks involving direct patient interactions, and functions as a vital member of the patient’s care team.

Summary of Duties and Responsibilities

  1. Greet patients arriving at the unit, introducing themselves with name and title.
  2. Use two patient identifiers to accurately identify each patient.
  3. Escort patients to exam rooms and prepare them for visits by taking vital signs, including height, weight, pain level, and pulse oximetry.
  4. Interview patients and families to gather personal and medical histories; provide information on healthcare services as needed.
  5. Complete screenings for depression, HIV, domestic violence, and smoking cessation.
  6. Assist providers during examinations by providing necessary equipment and supplies.
  7. Instruct patients on preparation for laboratory tests, procedures, and medical visits.
  8. Offer HIV testing to all patients aged 13 to 64.
  9. Obtain or instruct patients to obtain specimens for lab testing, such as stool or urine samples.
  10. Perform venipuncture for blood samples using two patient identifiers.
  11. Label all specimens in the presence of the patient.
  12. Verify collected specimens with requisitions and prepare them for laboratory transport.
  13. Prepare specimen containers and label specimens during procedures like PAP smears or pathology specimen collection.
  14. Perform point-of-care tests as ordered, document results in the EMR, and log sheets per policy.
  15. Monitor and record refrigerator temperatures in the lab, perform quality controls for POCT.
  16. Conduct hearing and vision screenings for appropriate patients or as ordered.
  17. Obtain peak flow readings as ordered.
  18. Operate, test, and maintain medical equipment for diagnostic and therapeutic procedures.
  19. Document all patient contact and information in the EMR, notifying nurses of out-of-range values.
  20. Observe and report changes in patient conditions to the care team.
  21. Assist with orientation and training of new staff members.
  22. Perform CPR, basic first aid, wound care, and EKG testing as needed.
  23. Participate in pre-visit planning, clinical huddles, and activities to meet Patient-Centered Medical Home standards.
  24. Identify care gaps and collaborate with the team to address them through outreach, education, and referrals.
  25. Coordinate care activities via Accountable Care Organizations (ACO) and other initiatives.
  26. Register and discharge patients in the EPIC system and EMR.
  27. Adhere to workflows and complete EPIC work queues as assigned.
  28. Respond to client inquiries, schedule appointments, and make referrals.
  29. Coordinate and schedule appointments across facilities, and contact patients for rescheduling as necessary.
  30. Validate PCP assignments and update as needed; make reminder calls for appointments.
  31. Prepare examination rooms, ensure supplies are stocked, and maintain cleanliness and safety.
  32. Assist with simple medical procedures, explain care to patients, and maintain confidentiality per HIPAA guidelines.
  33. Work effectively as part of the healthcare team and participate in ongoing education and training.
  34. Provide basic patient education on follow-up and referrals, emphasizing the importance of continued care.
  35. Demonstrate excellent customer service and participate in quality improvement activities.
  36. Utilize interpretation services as needed and follow universal precautions.
  37. Use safe body mechanics when assisting patients and participate in community outreach.
  38. Demonstrate age-specific competencies and uphold safety standards.
  39. Report hazards, incidents, or changes in patient conditions immediately to the appropriate personnel.
  40. Maintain professional conduct, adhere to dress codes, and complete mandated training annually.

Judgment and Decision Making

Communicate pertinent patient information, manage conflicts effectively, and escalate issues requiring follow-up or additional care. Report emergencies, equipment malfunctions, or hazardous conditions immediately. Maintain client confidentiality and demonstrate effective communication skills.

Relationships with Others

Establish positive, professional relationships with patients, visitors, and colleagues. Contribute to team meetings and patient satisfaction initiatives. Demonstrate cultural competence and commitment to quality care.

Knowledge of Job

Adhere to safety policies, demonstrate competency in first aid, wound care, and life support, and operate medical and technical equipment safely. Engage in continuous learning to improve patient care quality.

Safety Awareness

Report safety concerns, follow emergency protocols, and observe infection control standards. Maintain a safe work environment, adhere to policies, and wear appropriate identification and uniforms. Comply with all regulations and report absences or tardiness promptly.

Minimum Qualifications

High school diploma or equivalent, successful completion of phlebotomy and EKG training, and current BLS certification from the American Heart Association. For long-term care, NYS Nurse’s Aide certification is required.

Department Preferences

Qualifications:

  • EKG and Phlebotomy Certification
  • High school diploma or equivalent, with completion of NYS Department of Health training programs
  • Knowledge of Spanish or a second language preferred
  • Proficiency in English and another language
  • Effective interpersonal and customer service skills

Equipment/Software:

  • Manual and automatic blood pressure machines
  • Electronic Medical Record systems

Experience:

At least 1 year of ambulatory care experience preferred.
