ROTATING SCHEDULE
PURPOSE OF POSITION:
Under direct supervision of the Registered Professional nurse and other licensed nursing personnel, performs activities and procedures of a non-professional nature indirect/indirect care of patients care of patient and residents, or participates as a member of a treatment, nursing, medical, health care team in implementing appropriate patient care duties.
RESPONSIBILITIES:
(1) Performs or assist with all activities that are within the scope of the service-specific functional job description of:
• Patient Care Associate – surgical Technician
• Office Aide.
(2) Conducts interview of patients to obtain routine personal and medical information and appropriate health history.
(3) Obtains blood specimens for laboratory testing by performing venipuncture and/orfinger stick testing.
(4) Performs blood glucose testing after receiving training and annual retraining.
(5) Performs other waived testing procedure to the services after receiving training.
(6) Performs 12 Lead electrocardiography.
(7) Ensures a safe and secure environment free of hazards to reduce the risk of injury to all individuals (patients, visitors and staff).
(8) Provides patients/families with therapeutic care and reinforces patient/family centered care.
(9) Effectively utilizes the electronic medical record as well as the paper medical record to document all care delivered.
(10) Performs clerical duties such as open visits, preparation of clerical records and skills.
(11) Assists in the care of preparative preparation of patients:

(13) Assist in providing care for the medical patients with but not limited to COPD, Asthma DM, Renal, HIV HTN.
(14) To respond and differentiate clinical alarm system.
(15) Participates in Rehabilitation Activities and demonstrates skills in:
• Therapeutic positioning
• Range of motion exercises
• Supervises Transfers
• Ambulation and use of assistance devices
• Bowel/bladder training
• Supervises activities of daily living
(16) Delivers patient care services in a clinically competent and patient-centered manner as demonstrated by level of education, training and clinical competence that is inclusive of and not limited to the following diagnosis:
a)CHF
b) TB
c) Geriatric/Dementia patients
d) Stroke
e) Colon Cancer
f) Cholecystectomy
g) Head Trauma
h) Long bone Fracture
i) Hysterectomy
j) Brachtherapy
k) Use of Cyracom phone for LEP patients
l) MRI Safety
m) Cultural competence
(17) Performs all related duties as assigned.
Qualification Requirements:
1. High school diploma or its educational equivalent, approved by a State’s Department of Education or a recognized accrediting organization; and
2. Successful completion of training courses including the performance of phlebotomy and
electrocardiography (EKG), or completion of a training program or programs that include the performance of phlebotomy and EKG, such as Phlebotomy Technician and EKG Technician training programs, a Patient Care training program, or a Medical Assistant training program; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Special Note:
In addition to meeting the qualifications above, assignment to a long-term care facility requires New York State Nurse’s Aide certification.
