  • Assist assigned patients with activities of daily living per treatment plan and unit routines.
  • Observe, report, and document changes in patient status in a timely and accurate manner.

  • Escort patients to departments, meals, and other areas per protocol.
  • Perform assignments according to policy and procedures.
  • Provide Constant Supportive Observation (CSO) as assigned.
  • Participate in therapeutic activities or groups for patients.
  • Contribute to documentation and recovery-oriented care.

Job Duties
Patient Care
  • Assists assigned patients with activities of daily living (ADL) according to treatment plan, policies, procedures, unit routines and protocol.
  • Observe, report, document changes in the patient’s physical and behavioral status, ADLs, and other tasks as assigned to the licensed nurse or Lead CNA in a timely and accurate manner.
  • Utilize therapeutic interventions that reflect knowledge of Therapeutic Options unit routines and rules, and patient treatment guidelines.
  • Escort patients to other departments, meals and outside of hospital per protocols and procedure.
  • Perform assignments according to policy and procedures.
  • Constant Supportive Observation (CS0) as assigned.
  • Participates in therapeutic activities and/or groups for patients.
  • Performs assignments utilizing principles of Recovery Model
Operational Responsibility
  • Follows and completes assignments in timely manner. Reports on assignments to licensed nurse or Lead CNA as required.
  • Participate in activities that improve patient care, as directed.
  • Accurately completes required documentation within established time frames.
  • Assists with admissions, discharges, and transfers as assigned.
  • Compliance with established policy CHPP 12-03.10, Employee Reliability Standards.
Education/Skill Development
  • Attend all mandatory in-services.
  • Complete five hours of in-service to meet learning needs and developmental plan.
  • Shares learned knowledge and skills with staff members as appropriate.
Minimum Qualifications

Ability to provide patient care under supervision of licensed staff and/or Lead CNA; ability to follow instructions and report observations to licensed nurse in timely manner; skills in providing assistance to patients in activities of daily living.
