Job summary

CommonSpirit Health's Continuing Care Hospital in Lexington, Kentucky seeks a Patient Care Assistant to support patients with daily living activities, hygiene, mobility and feeding, while assisting RN staff with vital signs and room preparation. You will help create a safe, comfortable patient environment and participate in routine rounds and care tasks.

You will document vitals, observe patient conditions, and assist with admissions, transfers and discharge processes under supervision,

Qualifications

• High school diploma or GED required at hire.
• 1–3 years direct patient care experience.
• Basic Life Support (CPR) within 14 days.

Responsibilities

• Provide direct patient care as delegated by RN staff.
• Monitor and record vital signs.
• Assist with personal hygiene and physical care needs.
• Assist in admission, transfer and discharge processes.
• Prepare patient rooms and assist with unit housekeeping.
• Act as runner/transporter when needed.

As our Patient Care Assistant, you’ll help our patients feel safer and more comfortable by assisting with their basic daily life activities.

Every day, you will directly support patients with tasks such as personal hygiene, mobility, and feeding, while also assisting nursing staff with vital sign monitoring and preparing patient rooms.

Your success in this role comes from your genuine desire to help patients feel better, whether that means giving them fresh bed linens or checking their vitals. You're diligent about your tasks, observant of patients’ conditions improving or declining, and accept coaching well.

• Provides direct patient care as delegated by RN staff
• Monitors and records vital signs Assists in providing personal hygiene and physical care needs Accurately measures, records, and reports intake and output Obtains and records daily weight Performs blood glucose testing using blood glucose meter Reports changes in patient’s status promptly to RN Performs as patient care companion when needed Assists in the admission, transfer and discharge process Performs unit housekeeping needs Acts as a runner/transporter
• Checks and documents refrigerator temperatures as delegated, reporting variances to appropriate personnel
• Updates assigned bed status
• Maintains a safe environment by responding to all bed alarms, call lights, telephones and alerting the correct person
• Keeps patient environment clean, Performs comfort rounds, keeps hallways clear of obstruction, no food or drink at the workstation, reports safety issues and broken equipment appropriately, and adheres to patient safety precautions
