Job summary

UChicago Medicine AdventHealth in Glendale Heights, IL is seeking a Care Management coordinator to coordinate referrals and support patient care across the continuum. You will stay at the bedside as needed, ensure regulatory compliance, and join daily rounds to inform discharge planning and optimize outcomes.

Required is a High School Grad or Equiv; BLS and crisis intervention are preferred. Benefits start Day One, with a 7am-7pm rotating schedule, and a comprehensive benefits package is offered.

Qualifications

• High School Grad or Equiv required.
• BLS certification preferred.
• NonViolent Crisis Intervention Program preferred.

Responsibilities

• Coordinates referrals from interdisciplinary team members and facilitates patient care management across the continuum.
• Ensures quality patient care and regulatory compliance.
• Participates in daily multidisciplinary rounds to review progression of care and discharge plans for all assigned patients.
• Remains at the bedside of assigned patients at all times unless relieved by an appropriate staff member.
• Assists in calming and reorienting patients in cases of agitation or confusion.
• Removes hazards and broken equipment from the environment and reports as appropriate.
• Records observations accurately and promptly; supports patients with complex psychosocial needs under mentorship.
• Performs other duties as assigned.
