Job summary

Encompass Health is seeking a Patient Assessment Standards Coordinator to ensure accurate data extraction from clinical documentation and timely submission for Medicare patients. You will educate staff, provide primary support for IRF-PAI coding, and drive improvements in data quality and timelines.

The role emphasizes meticulous documentation, coordination across teams, and adherence to CMS/UDS deadlines to support quality indicators and patient care standards.

Qualifications

• Assure accurate data extraction from clinical documentation.
• Coordinate timely submission of data for Medicare patients.
• Educate and support staff on proper and accurate documentation.
• Act as the primary resource for problem-solving regarding Quality Indicator coding and IRF-PAI completion.
• Work to improve process of QI coding and data collection for IRF-PAI.
• Ensure IRF-PAI data is entered and transmitted accurately.
• Ensure IRF-PAI data is transmitted to CMS/UDS within required time frames.
• Review, interpret and collect data for IRF-PAI completion.
• Collaborate to identify comorbidities or accurate CMG/RIC categories.
• Ensure discharged patient records contain required elements.
• Complete chart audits for timely UDS export and CMS transmission.

• Collaborate with team on identification of potential comorbidities or accurate CMG/RIC categories.
• Ensure all discharged patient records contain the required elements.
• Complete chart audit to allow for timely UDS export and CMS transmission.
