Encompass Health Rehabilitation Hospital of West Valley seeks a Patient Assessment Standards Coordinator to ensure compliant, high-quality patient assessments. You will extract data from clinical documentation, coordinate Medicare submissions, and educate staff on accurate IRF-PAI coding.

Qualifications

• 1 year of healthcare experience.
• Attend all EHC IRF-PAI trainings.
• Preferred: rehab or post-acute care experience, teaching and communication skills
• Detail-oriented with deadlines, independent work ethic.

Responsibilities

• Assure accurate data extraction from clinical documentation.
• Coordinate timely submission of data for Medicare patients.
• Educate and support staff on proper and accurate documentation.
• Act as the primary resource for problem-solving regarding Quality Indicator coding and IRF-PAI completion.
• Work to improve the process of QI coding and data collection for IRF-PAI.
• Ensure IRF-PAI data is entered and transmitted accurately.
• Ensure IRF-PAI data is transmitted to CMS/UDS within specified time frames.
• Review, interpret and collect data on each patient for IRF-PAI completion.
• Collaborate on identifying comorbidities or accurate CMG/RIC categories.
• Ensure discharged records contain required elements.
• Complete chart audits for timely UDS export and CMS transmission.
