The Patient Assessment Coordinator uses discretion and independent judgment to coordinate the process of providing accurate and timely data for the Prospective Payment System and quality reporting for Medicare patients. Responsibilities include overseeing data collection on the IRF/PAI, ensuring supporting documentation, staff education for CMS compliance, reviewing IRF/PAI forms for accuracy and timeliness, monitoring scoring accuracy, and inputting information into the eRehabData system. Additional duties involve coordinating patient information collection, setting IRF/PAI completion dates, developing policies, and serving as the primary resource for Medicare IRF payment system issues.

Qualifications:

• Graduate of an accredited OT, PT, SLP, or RN program with current licensure in the state; ASN required; Bachelor’s degree preferred for nursing; Master’s degree for OT/PT/SLP; BLS/CPR card required. Alternatively, 3-5 years of experience in a patient assessment coordinator role may substitute for degree and license.
• 3-5 years of experience in patient care within an acute hospital, rehabilitation, or LTACH setting.
