Job summary

Piedmont Macon Medical in Macon, GA is seeking an IRF Patient Assessment Instrument Coordinator to support the Rehab Program Director. You will coordinate the timely and accurate completion of the PAI for each rehabilitation inpatient and provide clinical support to ensure proper scoring and care reflection.

This role requires an Associate’s degree (Bachelor’s preferred), 2 years as a licensed clinician (RN, PT, OT, or ST) in acute hospital or rehab settings, and active licensure.

Qualifications

  • Licensed clinician (RN, PT, OT, ST) with 2 years of acute hospital or rehab experience.
  • Ability to handle confidential patient information with discretion.

Responsibilities

  • Coordinate timely and accurate completion of the PAI for rehabilitation inpatients.
  • Provide clinical support to ensure accurate scoring of functional status and care needs.
  • Assist in monitoring inpatient benchmarks.
  • Maintain confidentiality of patient information.

Responsibilities

The IRF Patient Assessment Instrument Coordinator functions under the Rehab Program Director. Provides coordination for the timely and accurate completion of the Patient Assessment Instrument for each rehabilitation inpatient. This individual provides clinical support in completion of data contained in the PAI for each patient to ensure accuracy in the scoring of functional status and reflection of the patient's care needs. In addition, this position may assist in the ongoing monitoring of identified benchmarks for the inpatient population. The IRF PAI Coordinator will have access to confidential patient information and maintains confidentiality of all information.

Qualifications
Education
  • Associate’s Degree Required
  • Bachelor’s Degree Preferred
Work Experience
  • 2 years of experience as a licensed clinician (RN, PT, OT, ST) in an acute hospital or rehabilitation setting (IRF, LTAC, SNF, NH) Required
Licenses and Certifications
  • Must have at least one of the following: Upon Hire Required
  • RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required or
  • PT - Physical Therapist Required or
  • OT - Occupational Therapist – Registered Required or
  • ST - Speech Therapist Required or
