Legacy Home Health Agency, Inc. in San Antonio, TX is seeking a Patient Advocate who will be the face of the agency, helping families access home care services, equipment, and benefits while staying safe at home.

The role involves conducting home assessments, coordinating with doctors and DME providers, navigating Texas Medicaid applications, and converting inquiries into trusted client relationships while maintaining empathy and professionalism.

Qualifications

• 1–2 years in Home Care, Social Work, or Healthcare Marketing (Texas HCSSA experience is a plus)
• Familiarity with HHSC Form 2060, Medicaid PHC/CAS programs, and Medicare basics
• Must have a reliable vehicle and a valid TX Driver's License for daily home visits
• Bilingual: English/Spanish required

Responsibilities

• Conduct Home Assessments: Visit potential clients to assess their needs for PAS and determine eligibility.
• Be a Problem Solver: Identify when a patient needs more than just a caregiver—spotting needs for Home Health, Hospice, DME, or Meals on Wheels.
• Navigate the System: Assist families with Texas Medicaid applications and renewals via the Your Texas Benefits portal.
• Convert Leads: Turn inquiries into active clients by building trust and demonstrating how our agency improves their quality of life.
• Build a Network: Partner with local doctors, discharge planners, and DME providers to create a referral ecosystem.

• Be a Problem Solver: Identify when a patient needs more than just a caregiver—spotting needs for Home Health, Hospice, DME (walkers/hospital beds), or Meals on Wheels.
• Navigate the System: Assist families with Texas Medicaid applications and renewals via the Your Texas Benefitsportal.
• Convert Leads: Turn inquiries into active clients by building trust and demonstrating how our agency improves their quality of life.
• Build a Network: Partner with local doctors, discharge planners, and DME providers to create a referral ecosystem.

What We're Looking For:

• Experience: 1–2 years in Home Care, Social Work, or Healthcare Marketing (Texas HCSSA experience is a huge plus!)
