Compensation ranges from $45–$80 per hour, depending on licensure and qualifications.

POSITION SUMMARY
The Clinical Patient Access Specialist conducts structured outreach to established or assigned patients, administers standardized sexual health questionnaires, documents patient responses, identifies barriers to care, and facilitates appropriate follow-up and connection to services.
This position may be filled by an LPN, RN, APRN/NP/PA, or physician. Responsibilities will be assigned in accordance with the selected candidate’s professional license and legally permitted scope of practice.
ESSENTIAL RESPONSIBILITIES
- Conduct confidential sexual health assessments, obtain relevant health histories, perform risk assessments, and complete clinical evaluations appropriate to the individual’s licensure and assigned role.
- Provide evidence-based education, counseling, prevention planning, and shared decision-making in a culturally responsive, medically accurate, trauma-informed, and LGBTQ+ affirming manner.
- Conduct follow-up outreach related to results, treatment, medication adherence, missed appointments, and retention in care.
- Identify patient access barriers and help connect patients with practical resources and solutions.
- Accurately document all patient call attempts, outcomes, and follow-up needs in the designated call tracker and applicable patient record.
- Meet established call-volume, quality, and productivity expectations while complying with the current Patient Call Policy.
- Complete assigned calling shifts and follow all scheduling, documentation, patient-contact, and escalation requirements.
- Support relationship-building with community organizations, healthcare partners, industry groups, and other stakeholders.
- Collaborate with Compliance, Finance, Human Resources, Information Technology, and other internal teams to achieve organizational objectives.
REFERRAL, LINKAGE, AND CARE COORDINATION
- Refer and connect patients to HIV and STI testing services through Nashville CARES and/or CARES 360 Clinic at 633 Thompson Lane during Phase 1.
- Once the on-site STI testing facility at 100 Oaks opens, coordinate referrals to the appropriate provider at that location during Phase 2.
- Accommodate patient requests for referral to an alternative qualified testing provider when appropriate.
- Coordinate referrals for new-patient establishment and ongoing care with the Vanderbilt Comprehensive Care Clinic, Nashville CARES/CARES 360 Clinic, and other community healthcare providers.
- Facilitate warm handoffs, assist with appointment scheduling, and confirm that patients are successfully connected to the referred service or provider.
- Serve as a consistent point of contact for patients navigating the sexual health and HIV/STI care continuum.
- Identify barriers involving cost, transportation, scheduling, stigma, fear, or other access challenges and connect patients with Nashville CARES, Vanderbilt, or additional community partners for support.
DOCUMENTATION, CONFIDENTIALITY, AND COMPLIANCE
- Maintain accurate, timely, and confidential documentation in the electronic health record and/or patient navigation system, including eClinicalWorks.
- Comply with HIPAA and all applicable federal and state privacy and confidentiality requirements, including 42 CFR Part 2 when applicable.
- Practice strictly within the scope of the individual’s professional license and all applicable laws and regulations.
- Follow established procedures for clinical escalation, patient safety, documentation, and continuity of care.
- Support required program data collection and reporting, including applicable quality, grant, Ryan White, and 340B program metrics.
REQUIRED QUALIFICATIONS
- Active, unrestricted license or authority in at least one applicable tier: LPN, RN, APRN/NP, PA, MD, or DO. Advanced practice clinicians must also maintain any required national certification.
- Excellent communication, interpersonal, and telephone skills.
- Strong typing, documentation, and data-entry skills.
- Ability to use Microsoft 365, video-conferencing tools, electronic health records, and call-tracking systems.
- Ability to work efficiently in a fast-paced environment while maintaining positive and respectful patient relationships.
- Positive, energetic, flexible, and professional approach to patient care and teamwork.
- Ability and willingness to travel locally, regionally, and nationally when needed to support community engagement, communications, and organizational activities.
- Successful completion of all required background checks, exclusion screenings, credentialing, education and employment verification, and drug-screening processes, as applicable.
- Ability to perform additional duties as assigned based on organizational and program needs.
PREFERRED QUALIFICATIONS
- One to three years of experience in patient outreach, ambulatory care, telehealth, sexual health, HIV care, public health, care coordination, or patient access.
- Experience serving people living with HIV, LGBTQ+ communities, communities of color, and patients affected by stigma or barriers to care.
- Knowledge of PrEP, PEP, Doxy-PEP, HIV/STI prevention and treatment, Ryan White programs, federally qualified health centers, or the 340B Drug Pricing Program.
- Experience working with a nonprofit organization or 340B-covered entity.
- Demonstrated ability to build rapport and provide culturally responsive, nonjudgmental, and affirming support in ethnically, culturally, and racially diverse environments.
- Foundational knowledge of—or willingness to learn—concepts and terminology relevant to LGBTQ+ communities, including pronoun usage, sex, sexual orientation, gender, gender identity, and intersectionality.
- Ability to align clinical activities and patient-access initiatives with broader organizational goals.

COMPENSATION
- LPN: $45 per hour
- RN: $55 per hour
- APRN/NP/PA: $70 per hour
- MD/DO: $80 per hour
Additional certifications may result in an incremental increase to the hourly rate at NPS Healthcare’s discretion.
