Holyoke Visiting Nurses Association, Inc. is seeking a Patient Accounts Coordinator for a part-time, 24-hour-per-week role on the day shift (Monday–Friday) in Holyoke, MA.

The position emphasizes triage, referrals, and coordination for home health and hospice services, with strong customer-service expectations. Qualified candidates are RNs or LPNs licensed in Massachusetts, with at least three years of nursing experience including home care or hospice.

Competenze

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current MA licensure.
• Minimum of three years of nursing experience with at least one year in Home Care and/or Hospice.

Mansioni

• Obtain a referral for services, including patient information, diagnosis, MD order with specifics for direct care.
• Discuss home health services and hospice services with referral sources and identify available services.
• Perform an initial assessment of patient needs for home care and hospice services to determine Medicare/Medicaid or other payer criteria.
• Evaluate all patient referral information for completeness and signatures; follow up with referral sources for missing information.
• Input patient referrals and related information into Clinical Link and other systems.
• Offer alternative settings when home care is not appropriate (outpatient mental health providers, SNFs).
• Coordinate with Clinical Operations for assignment of initial home assessments and advising on level of need.
• Communicate patient needs clearly to supervisors, referral centers, and clinical staff.
• Answer patient and physician calls to obtain orders or provide direction on VNA care and triage.
• Process VNA interim orders via fax/phone/email and enter into EMR; alert clinical operations of changes.
• Perform other similar duties and establish positive relations with customers.
