$1,000 Hiring Bonus! $500 after 3 months, remaining $500 after 6 months.

Job Summary:

As a Case Manager, you will provide case management services to individuals currently in care or former program participants living independently. You will coordinate with county social workers, foster parents, probation, and other community agencies to develop and implement transition and housing plans. Responsibilities include developing and facilitating independent living skills and parenting groups, conducting bi-weekly program meetings, and supporting housing and life skills for participants. You will also market services and compile reports for program management.

Job Requirements:
  • Bachelor's degree in social service or related field and at least one year of experience with youth, life skills training, and group facilitation.
  • Valid driver's license, acceptable driving record, reliable transportation, valid insurance, and a Motor Vehicle Check.
  • Strong organizational skills.
  • Ability to focus and be attentive for extended periods.
  • Ability to develop, guide, and follow up on client transition plans.
  • Ability to promote a positive attitude about LSS and related programs.
  • Knowledge of community programs and referral processes.
