Job summary

Colorado Coalition for the Homeless seeks a dedicated PATH team member to assist individuals experiencing homelessness in achieving housing, benefits, and essential services. You will work closely with behavioral health clinicians to provide integrated, trauma-informed support and coordinate access to resources across partner agencies.

The role emphasizes client-centered advocacy, data entry in HMIS/EHR, and collaboration to improve outcomes for people experiencing homelessness in Colorado.

Qualifications

Responsibilities

  • Assist clients experiencing homelessness in achieving housing, benefits, legal aid, medical/mental health referrals, identification, and other needs.
  • Collaborate with PATH team and behavioral health clinicians to provide integrated services.
  • Complete documentation and data entry in HMIS and EHR in a timely, HIPAA-compliant manner.
  • Advocate for clients and coordinate with community partners to access resources.

Job description

This position will be posted for a minimum of 5 days

The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

This position works as a member of the PATH team to assist clients experiencing or at risk for experiencing homelessness in working towards client identified goals such as attaining needs such as housing, public benefits, legal assistance, medical/mental health/substance treatment referrals, identification, and other needs related to transitioning from homelessness. This position works collaboratively with the behavioral health clinicians on the PATH team to offer an integrated supportive approach when working with clients. This position develops working relationships within and outside of the organization to improve client access to needed services.

NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

This position works approximately 8 hours per day, 5 days a week (Monday through Friday), in person at both office and field locations. This position is based at 2100 Stout St, Denver, CO, 80205.

  • Valid driver's license required. This requirement may be waived, if necessary, based on overall candidate experience and current needs of the business and/or role.

  • Works as a member of the PATH team to provide face-to-face services to individuals experiencing homelessness (or at risk of experiencing homelessness) which at times, may require meetings outside of the office. Assists clients in recognizing and defining their own goals, inform them of available services and help them access goods, services, financial benefits, ID's, housing, and other needs.
  • Works collaboratively with homeless service providers in the community and CCH programs to provide optimum coordination, communication and continuity of services.
  • Provides supportive services to clients from a trauma-informed, culturally humble lens.
  • Advocates for the needs of individual clients as well as systemic change, whenever needed for the overall benefit of people experiencing homelessness.
  • Builds relationships with clients with the goal of housing, benefits and other resources needed to end clients' homelessness.
  • May transport homeless individuals using a CCH vehicle or an approved personal vehicle.
  • Accepts referrals from CCH staff and other agencies and works collaboratively with staff of all agencies to provide high quality service.
  • Meets with clients through a integrated intake process and sets individual appointments with clients ongoing as needed.
  • Completes all necessary documentation and electronic data entry requirements in both HMIS and EHR in a timely manner and in compliance with all relevant HIPAA regulations.
  • Assesses client needs, develops short and long-term service and treatment plans.
  • Coordinates on-going clinical and case management services to individuals.
  • Education: High School Diploma or GED required. Bachelor's degree in psychology, Social Work, Human Services, Criminal Justice preferred.
  • A combination of education and experience may be substituted for position requirements on a case-by-case basis.
  • Experience: 1-2 years of experience working with a diverse population who are experiencing mental health challenges, particularly those individuals diagnosed with serious and persistent mental illness (SPMI) preferred.
  • Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders preferred.
  • Strong understanding of housing first principles, trauma-informed care, and harm reduction strategies.
  • Excellent interpersonal and communication skills, with the ability to build trusting relationships with residents.
  • Ability to manage a diverse caseload, prioritize tasks, and work independently while also being a team player.
  • Proficiency in Microsoft Office Suite.
  • Knowledge of local housing resources, benefits programs, and social service agencies preferred.
  • Bilingual or multilingual skills are preferred.
