Vensure Employer Solutions is seeking a Participant Care Associate (CNA) for the PACE program at Hudson Headwaters in Glens Falls, NY. The role focuses on providing direct personal care to older adults within an interdisciplinary healthcare team.

The ideal candidate holds NYS CNA certification, a HS diploma or GED, and is comfortable with routine caregiving tasks, transfers, and documentation. Schedule is full-time, Mon-Fri, 8:00 AM–4:30 PM, with no weekends required.

Qualifications

• Active NYS CNA Certification.
• High School Diploma or GED.
• Experience or comfort providing hands-on personal care to elderly.
• Ability to lift up to 50 lbs.
• Reliable transportation.
• Ability to pass medical clearance and background checks.
• Strong communication and documentation skills.

Responsibilities

• Provide hands-on personal care (bathing, grooming, dressing, toileting).
• Assist with feeding and meal support.
• Assist with transfers and ambulation.
• Monitor participant well-being and report concerns.
• Complete participant care documentation.
• Support participant independence and safety.
• Follow individualized care plans and collaborate with interdisciplinary team members.
• Maintain participant dignity and comfort.

Pay~ $22.50/hour

About the Role~ Provide direct personal care and support to older adults enrolled in a Program of All-Inclusive Care for the Elderly (PACE), helping maintain safety, dignity, independence, and quality of life while working as part of an interdisciplinary healthcare team.
