Job summary

Vensure Employer Solutions is seeking a Day Center Aide – Personal Care & Participant Support for the PACE program at Hudson Headwaters in Glens Falls, NY. This full‑time role runs Monday through Friday, 8:00 AM to 4:30 PM, with no weekends and non-exempt status.

The position emphasizes hands‑on participant support, mobility assistance, meals, activities, and escorting participants to appointments to help maintain independence and dignity.

Qualifications

• High School Diploma or GED required.
• Personal Care Certification or ability to meet training requirements.
• Home Health Aide (HHA) certification accepted.
• Ability to complete competency training before working independently.
• On-site work Monday–Friday, 8:00 AM–4:30 PM.

Responsibilities

• Assist participants with daily activities and personal care.
• Provide participant supervision and safety monitoring.
• Assist with mobility, ambulation, and transfers.
• Support recreational and social activities.
• Assist with meal service and feeding support.
• Escort participants to appointments as needed.
• Encourage independence and dignity; communicate concerns to the care team.
• Assist with documentation and reporting; maintain a safe environment.

• Pay~ $21.50 per hour
• Schedule~ Full-Time
• Monday–Friday
• 8~00 AM–4~30 PM

• About the role~ Provide hands‑on participant support and assist older adults throughout the day center with mobility, supervision, activities, meals, and appointment escorts to help maintain independence, dignity, and quality of life. Training is available.
• Key responsibilities~
