Job summary

CareMed is seeking a Wound NP to provide comprehensive wound care for residents in long-term care facilities. You will diagnose, treat, and manage chronic wounds, coordinate with facility staff, and collaborate with primary care physicians to optimize outcomes.

This role requires Kentucky NP license, board certification, and experience with wound care and wound vacs; DEA license optional. You will work as part of an interdisciplinary team and use EMR systems.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure (Optional).
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including chronic, debilitating, stalled, or infected wounds.
• Diagnose and treat diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Staging pressure injuries according to the NPIAP.
• Familiar with wound care products; collagens, alginates, foams, silvers, etc.
• Experience with wound vacs and DME.
• Wound certification preferred or openness to getting certified.
• Experience in long-term care setting.
• EMR proficiency.
• Interdisciplinary care team coordination.

Responsibilities

• Prescribe medications, including controlled substances per KY law.
• Order, review, and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff about changes in condition.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.
  

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
