Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to provide wound services to residents in long-term care facilities in Kentucky. You will join an interdisciplinary team to assess and manage wounds, coordinate care plans, and communicate with facility staff on changes in patient condition.

Requirements include an active Kentucky NP license and board certification. Experience with wound care, wound vacs, and DME is preferred, with a willingness to obtain additional wound

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including chronic, debilitating, stalled, or infected wounds
• Must diagnose and treat diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries per the NPIAP
• Familiar with wound care products; collagens, alginates, foams, silvers, etc.
• Experience with wound vacs and other DME, such as pressure-relieving/reducing devices
• Preference for wound certification or willingness to obtain certification
• Experience in long-term care setting
• Ability to work with an EMR
• Ability to work within an interdisciplinary care team to coordinate patient care

Responsibilities

• Provide wound care services for long-term care patients.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as-needed basis to discuss changes in condition.
• Coordinate with the care plan and the primary care physician.

• Provide wound care services for long-term care patients residing in skilled nursing facilities.

• Prescribe appropriate medications, including controlled substances in accordance with KY Law.

• Order, review, and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Experience working in a long-term care setting.
