Job summary

Woundtech in San Antonio, TX seeks a Nurse Practitioner or Physician Assistant for part-time home-visit wound care. You will provide hands-on care, perform debridement, and manage wound treatment plans. Travel to patient homes is required with documentation of visits and coordination with supervising physicians.

Must have NP or PA certification, active Texas license, 1–3 years of experience, and ability to lift 50 lbs. Driver’s license and strong communication skills are essential.

Qualifications

• Graduate nursing degree (NP) or PA with certification.
• Active NP/PA license in state; wound care cert preferred.
• 1–3 years of advanced practice experience.
• Ability to practice autonomously and lift 50 lbs.
• Valid driver's license; ability to travel for home visits.
• Excellent verbal and written communication; organized and detail oriented.

Responsibilities

• Provision of hands-on care to patients during home visits.
• Debridement of wounds by various methods performed safely.
• Application of topical wound care and wound management products.
• Recommend services to improve wound healing (labs, radiology, referrals, home health, VAC device, orthotics).
• Interpret diagnostic tests and follow up as needed.
• Recommend prescriptions and DME to aid healing; within scope and supervising physician agreement.
• Conduct risk assessments for Members with Diabetes per ADA guidelines.
• Educate patients and communicate with PCP/CM/physicians; provide updates.
