Job summary

Day Kimball Health is seeking a part-time Advance Practice Provider (APP) to join the Wound Care Clinic in Putnam, CT. The APP will deliver comprehensive evaluation, treatment, and ongoing management of patients with acute and chronic wounds in an outpatient setting, collaborating with physicians and a multidisciplinary team.

Key responsibilities include wound assessment, developing evidence-based treatment plans, and bedside procedures within scope of practice.

Qualifications

• Minimum of 3 years of clinical APP experience preferred.
• Wound care experience strongly preferred.

Responsibilities

• Evaluate patients with acute, chronic, surgical, diabetic, venous, arterial, and pressure wounds.
• Perform comprehensive assessments, including vascular, neurological, and infection evaluations.
• Develop and implement individualized treatment plans using evidence-based wound care practices.
