Hearthside Senior Living Bartlett

Nursing that fits your life, not the other way around.

Looking for steady part-time hours where your skills are valued and you actually get to know your residents? Hearthside is a smaller, more personal Assisted Living community, not a hospital and not a skilled nursing floor. Here, you'll build real relationships with residents and families, your clinical judgment is trusted, and your team has your back.

The Wellness Nurse supports the delivery of resident care and services in accordance with applicable regulations, accepted standards of nursing practice, and Allegro Living policies. This role provides clinical support, administers medications, monitors resident wellness, supports team member supervision, completes documentation, and communicates with residents, families, physicians, pharmacies, and outside providers.

The Wellness Nurse promotes a safe, supportive, resident-centered environment while helping ensure timely documentation, quality outcomes, and compliance with Company policies and state requirements. The ideal candidate is compassionate, organized, collaborative, and committed to serving seniors with professionalism and sound nursing judgment.

Key Responsibilities
  • Provide daily supervision and guidance to assigned non-licensed team members and other designated personnel in meeting residents’ needs.
  • Monitor resident wellness, identify changes in condition, and respond appropriately to medication needs or other circumstances requiring additional care.
  • Provide resident care in accordance with provider orders, regulatory requirements, recognized nursing practice, established standards, and Allegro Living policy.
  • Accurately administer prescribed medications and support the continuity of resident care.
  • Make work assignments, support team members, and help identify opportunities to improve resident care and services.
  • Communicate effectively with residents, families, team members, physicians, pharmacies, professional consultants, and third-party providers to support coordinated care.
  • Provide first aid as required and complete assigned duties in accordance with Company procedures.
  • Participate in required meetings and in-service training sessions, including Relias training, in accordance with state guidelines and Company requirements.
  • Support compliance with residents’ rights requirements and report observed or suspected violations immediately.
  • Perform other related duties as assigned by the supervisor.
Qualifications
  • Licensed Practical Nurse in good standing with the applicable state licensing agency, as required by state regulations.
  • CPR/AED certification required.
  • Minimum of one (1) year of experience as a Licensed Practical Nurse in a senior care setting preferred.
  • Previous experience with computerized resident documentation systems preferred.
  • General understanding of, and concern for, the needs of seniors.
  • Experience supporting residents with Alzheimer’s disease, dementia, or memory impairment preferred.
  • Ability to work well with others and promote a team-oriented environment.
  • Ability to maintain confidentiality and professional composure in stressful or challenging conditions.
  • Must be at least 18 years of age. State-specific guidelines may apply.
  • Valid driver’s license and acceptable Motor Vehicle Record in accordance with Company requirements, when driving responsibilities are assigned.
What Makes You Successful in This Role
  • You provide compassionate, professional care while using sound nursing judgment.
  • You communicate clearly with residents, families, team members, physicians, pharmacies, providers, and community leadership.
  • You stay organized, follow through on documentation, and help maintain compliance with Company policies and state requirements.
  • You support teamwork and accountability while modeling resident-centered care.
  • You balance daily resident needs with clinical, regulatory, documentation, and communication priorities.

Join Allegro Living and help us provide exceptional, resident-centered care while making a meaningful impact across our communities.
